Thank you for your interest in the EagleTech Partners Annual Compliance Questionnaire and Outside Business Activities Surveys.  The following information is used by our organization to configure your customized survey and Survey Analysis Report.

Survey Specific Information:
We will configure your survey based on the following information.

Survey Open / Close Date:
Please select an Open and Close Date for your Survey.  Typically we recommend leaving the survey open for at least 1 month.

Weekly Report Day of Week:
Please select a day of the week your reporting officer would like to receive a Survey Analysis Report.  By default we normally send out a report on Tuesday of each week the survey is open.

Once you click submit, your responses will be sent to EagleTech Partners.  A representative will be contacting the individual you identified above for payment details.  Once payment is received, your survey will be opened for completion and an email will be sent to each of the individuals you identified above.
